St.
Johns County Board of County Commissioners is seeking a MDT Facilitator Case Manager for our well-established Community Based Care Program, the Family Integrity Program, which is a division of the St.
Johns County Health and Human Services Department whose headquarters are located in beautiful St.
Augustine, Florida.

Johns County Family Integrity Program’s mission is to work in partnership with the State of Florida and the local community to develop, implement and manage a highly effective, strength-based community-based system of care for abused and neglected children and their families.
The Family Integrity Program provides Dependency Case Management, Family Preservation, Outreach, Foster Care Licensing, Adoption and Independent Living Services for youth aging out of foster care.
Employees work in a team environment across the various programs with a consistent focus on child safety, child well-being and permanency.
Examples of Duties: The ideal MDT Facilitator Case Manager candidate must possess St.
Johns County’s core values of compassion, innovation, and trustworthiness and must work well in a collaborative team environment.
This position is responsible for performing case management duties in support of assigned clients and programs.
Additionally, this position is responsible for: * Prepares and maintains complete and accurate case records and reports.
Participates in supervisory review of cases through periodic conferences.
Collaborates with the care team to develop and recommend appropriate plans of services.
* Provides referrals to local resources.
* Reviews standard operating procedures to determine the best method to provide services to clients.
* Assists clients by coordinating transportation to needed services.
* Conducts home, school, and work visits.
* Completes telephone contacts with clients, service providers, and investigators.
Collaborates with other service providers and safety monitors to ensure the health and welfare of children in care.
* Documents client contacts.
* Completes home visits to ensure compliance with policies and procedures.
* Transports foster children to appointments as needed.
* Maintains travel logs.
* Prepares judicial reviews and other administrative reports for the courts.
* Attends required trainings.
* Maintains a full-time presence on the job.
* Responds to after-hours emergencies.
* Performs other duties as assigned.
Typical Qualifications: Candidates for MDT Facilitator Case Manager are encouraged to apply if they meet the following minimum qualifications: * High school diploma or GED equivalent and four (4) years of law enforcement experience or active military service (honorably discharged or status that current service reflects serving honorably); OR an Associate’s Degree or 60+ credit hours from an accredited college or university and two years of professional work experience or two years of law enforcement experience or active military service; OR a Bachelor’s Degree from an accredited college or university.
* Experience sufficient to thoroughly understand the major duties of the position, usually associated with one to three years’ experience or service in the social services field.
* Must maintain certification by the State of Florida as a Child Welfare Professional.
May be hired as Case Manager Trainee and placed at a Pay Grade 15, and must obtain certification within twelve (12) months from the date of hire.
* Must possess and maintain a valid Florida driver's license and any other endorsements necessary to legally operate vehicles used while assigned to this position.
* Must possess good interpersonal and communication skills in order to serve others.
* Must be able to comprehend, speak and write the English language.
